Register new students:

At the time of registration, you must bring:

Residency verification can be provided by one of the following:


Withdraw a student:
Contact school to which student is assigned.  Sign request for release of official records.
 
Have medication administered:
Submit a note from a physician stating the name of medicine, dosage/time to be administered and possible side effects.  Complete a school form authorizing principal/designee to administer the medicine.  An adult must bring the medication to school in its original container.  In the event of a school sponsored activity off the school grounds, the parent/guardian or an adult designated by the parent must give the prescribed medication if the student cannot administer medication to himself/herself.  New state regulations do not allow school staff other than a nurse to administer medication.
 
Withdraw student during the school day:
Report to Main Office.  Parent/Guardian must sign out student.  Any other adult must have written permission from parent.
 
Resolve bus questions:
Phone Transportation Supervisor at 855-4640.
 
Obtain permission for use of school buildings of grounds:
Phone Director of Facilities and Operations for Building Use Form at 855-4610 or obtain forma at any school building main office.
 
Qualify for Free or Reduced Lunch:
Phone Food Service Cook Manager at 855-4627.
 
Resolve a school related problem:
  1. Call teacher when problem relates to the classroom.
  2. Call building principal if item relates to an incident in a particular building.
  3. Call Asst. Superintendent for Curriculum and Personnel (855-4600) for instruction/personnel issue.
  4. Contact Supervisor of Transportation with bus problems at 855-4640.
  5. Contact Superintendent if you have not been able to resolve problems at the building level.
  6. Contact the Board of Education if you are unable to resolve problems at the Superintendent's level.
